- Curriculum Vitae
- Three letters of recommendation, including a letter from current program director
- USMLE or COMLEX Transcript
- Medical School Transcript
- MSPE
- ECFMG Certificate, if applicable
- Letter attesting to General Psychiatry Board Eligibility (Training Documentation Form)
