Employers Trying to Verify Residency Training
- Confirm the CITY where the physician trained. There are several UAMS-affiliated Family Medicine programs in Arkansas, and each one keeps separate files.
- Confirm the NAME the physician used while in training. (We often are stymied in looking for records when the request contains a nickname or a different surname due to marriage, etc.)
- Contact the program in the city where the physician was in residency training.
- If it was in Little Rock, use the form below.
- If it was somewhere else, use this link to find contact information for the other programs. The form below goes only to the Little Rock program and will not be helpful to you if the physician trained in another city in Arkansas.
